  • VENDOR INFO | Check for a Lump

    6TH ANNUAL PINK OUT 5K VENDOR INFORMATION BOOTH SET UP Please bring all items that you will need for the setup and display of your booth. We highly recommend bringing a tent, table, a couple of chairs, and a cart to wheel all your items. We do not require a fee if you are selling goods, we just ask that you consider a donation. We will have plenty of water and a few food trucks on-site to purchase snacks, coffee, smoothies, etc. 7:00AM – 8:00AM Saturday, October 1st 2022 Steele Indian School Park 300 E Indian School Rd Phoenix, AZ 85012 Please arrive with enough time to set up your booth before the vendor village opens at 8AM. Parking: Please enter the park at 3rd St and Indian School Rd. Do not park in the VA parking lot (go past their parking). Volunteers will be on-site to help direct you. Refer to map attached. Upon Arriving you will need to check-in to get your booth space number, vendor agreement, and raffle tickets to hand out. Booth Spot: Volunteers will be on-site to give you your designated booth space number, and help you find the correct spot. We have many vendors, so it is very important that you set up in your correct booth space. There will be a large map printed out with all booth spaces and coordinating numbers for your reference. I have also attached a map that shows where you should enter, park, and the location of Vendor Village. Vendor Agreement: Please disregard any prior emails about signing a vendor agreement before the event. We will have a printed out vendor agreement that we will give to you when you arrive to sign. You will receive your copy through email. VENDOR VILLAGE RAFFLE This year we are doing a raffle, please hand out one raffle ticket (provided for you at check-in) to each person who visits your booth (limit 1 per person). If you would like to contribute to the prize basket, please let me know! How it works- Participate in our raffle through visiting each of the vendors! Collect a raffle ticket for each vendor you visit, the more tickets you have, the higher your chances are to win! Visit our Check for a Lump booth for the opportunity to participate in our Disneyland Raffle! We will have food and coffee trucks on site. Raffle winners will be called after the PINK OUT 5K. VENDOR VILLAGE TIMELINE 7:00 AM - Vendor Village Setup 8:00 AM - Vendor Village Opens 9:00 AM - Race begins, participants will begin the race, often there are a decent amount of non-participants that walk through vendor village during this, and is a great time to make connections with local vendors. We hope to see you come by our booth to say hi! 10:00 - 11:00 AM - Race ends, clean up begins. You may stay as long as you wish, there is no set "done time." Clean Up Please help to make sure to we leave our public park cleaner than we arrived by cleaning up your booth space. We will have volunteers on-site to help with our PINK OUT 5K clean-up, as well as large dumpsters with scheduled trash pick-up. SHARE! SHARE! SHARE! Clients apply online and, once approved, may select a wig of choice up to $300 at one of our approved wig shops, and Check for a Lump pays the bill. We are currently assisting approximately one woman a day - over 1,950 wigs provided since inception. “I received a beautiful looking wig!! I didn’t realize how much it would improve my thoughts and help me keep strong” Shawna "In June I found a large lump in my left breast. I assumed since I had a clean mammogram in November 2020, it probably was just a milk duct cyst... I was officially diagnosed with Triple Negative Invasive Ductal Carcinoma." Berklee Mammogram Program Providing free mammograms and all diagnostic testing necessary for diagnosis of breast cancer to qualifying women in Arizona. Clients apply online and, once approved, Check for a Lump is directly billed by the imagining company partners. We also host mammogram events through mobile mammography units. We have helped our clients cover over $ in mammogram and diagnostic testing expenses. Super Survivors Creating a welcoming space to connect breast cancer patients and survivors, along with their co-survivors/caregivers, through group therapeutic activities. Women can sign up for our free Super Survivor Unite events on our website. We have also added Super Survivor Kits that are available for women undergoing treatment as a way to lift their spirits and give them comfort during what can be a difficult time. Super Survivor Kits Lifting spirits and providing non-toxic essentials, our kits are available to any Arizona resident undergoing breast cancer treatment. Our Story In 2009, at the young age of thirty-nine, I was reminded by my childhood friend Shelley Sakala, with a playful posting on Facebook to perform a self-breast exam. I did and discovered a lump that turned out to be breast cancer. I was very fortunate in finding my cancer in the early stages and was able to save my breasts and, more importantly, my life. I endured nine months of treatment, including two surgeries, 4 months of chemotherapy and 2 months of radiation, During that time my family and me were showered with kindness, compassion and generosity by our community. We had meals delivered to our home for weeks at a time by friends and strangers. We had friends show up at our doorstep with yellow gloves on ready to scrub our floors. We had neighbors bring by wigs, cards, flowers, and people offering to help with our kids. Not only did I have my physical community but I had an online community on Facebook as well. There was always someone cheering me on and cheering me up when I was at my lowest moments. I was so grateful for every act of kindness that I decided I wanted to do something to give back to my community. I must give God this credit as well, as I felt like he was pushing me on to a path that He wanted me to follow. I also wanted to ensure that I was leaving a legacy that my daughters could be proud of just-in-case . . . and so with the help of another childhood friend, Kate Kunberger, I started Check for a Lump. Our mission was simple. We wanted to motivate other women to perform breast self-exams in the hopes of detecting breast cancer in the early stages. Shortly after our non-profit was founded we discovered that our foundation had great potential. We chose to implement a second program with our funds to directly assist breast cancer patients. Our Wig Out program provides free wigs to women undergoing chemotherapy for breast cancer treatment in Arizona. As time went on we continued to educate ourselves and, in doing so, discovered the need for education regarding prevention of breast cancer in our community. We have taken our educational outreach program to the next level with the help of our sponsors, community grants and supporters. We have also formed great relationships with other non-profits, hospitals, doctors, and breast cancer related businesses in our community in order to more efficiently achieve our goals. In the short amount of time we have been in existence, we have established our foundation as a prominent figure in the breast cancer community. We could not have achieved any of this with out the amazing support of our family, friends, and community. Continually, I’m overwhelmed by the generous acts of kindness that surrounds us.     Misión e Historia Holly Rose never dreamed of founding a non-profit until she battled breast cancer and received endless acts of kindness from her community to help her and her family get through a terrifying and difficult time in their life. Wanting to give back and create a legacy her daughters would be proud of she founded the non-profit Check for a Lump with the help of her childhood friends. Holly’s passion and unwavering commitment to educate and assist has been honored with many esteemed awards including: Phoenix Business Journal Health Care Hero Award Category Education Finalist, Arizona Foothills Magazine Best of 2017 Most Influential Woman in Valley Non-Profit, Trendsetter in the Valley and one of nine women honored for Women Who Move the Valley. 2016 Top 25 MASK Unity Moms Making a Difference Award 2016, Nominee for the Beth McDonald Woman of the Year Award 2014. In addition, Holly is a self-published author: Live and Give: Facebooking My Way Through Breast Cancer. Holly’s proudest accomplishment outside of work is her marriage of twenty-four years to her one and only true love, John, and their two incredible girls Hannah and Ivy. Having faced her mortality she is extremely grateful for every day and treasures every memory made with her family and friends. She gives thanks to God for keeping her alive and placing her on a beautiful and rewarding path. Holly Rose Founder and CEO Tiffany House Board President Tiffany House is a Chartered Advisor in Philanthropy (CAP) and financial advisor dedicated to Charitable Planning and helping her clients create a master game plan that incorporates their intellectual, ethical and financial legacy. She is passionate about educating and empowering families regarding financial matters to help them create a legacy of their own design and steward wealth to future generations while considering civic responsibility. She founded the Gift Planning Institute that provides outsourced planned giving services to non-profit organizations, gift planning strategies for professional advisors and match-making for donors. Tiffany’s experience helping clients donate complicated assets tax efficiently provides a unique opportunity for organizations to benefit from her vantage point. She enjoys collaborating with tax, legal, financial and business transition advisors, but favorite part of consulting is connecting donors to organizations that ignite their passions! Kate Kunberger JD Co-Founder/Member Kate is an attorney specializing in entertainment-related intellectual property litigation. She earned her B.A. from University of Arizona, and her J.D. from New York University, School of Law. Kate has a passion for international travel and has been to over 50 countries. Shelly Sakala Co-Founder/Treasurer Shelley Sakala is about as close to a native Phoenician as they come. She has lived in the valley since she was three years old and has been friends with our founder, Holly Rose, since grade school. Shelley has lived in other parts of the U.S. while working as a broadcast meteorologist and reporter. Her goal was always to land a job back in one of the largest television markets in the country…Phoenix! She did just that and worked for ABC 15 for more than five years before deciding to transition into real estate. Her television experience and extensive knowledge of the entire valley has helped her build a strong referral-based real estate business. Her connections also make here a valuable asset to Check for a Lump. Shelley is the proud mother of two little girls, Sage and Chloe, and wife to husband Joe. Erin Alstad Secretary Mrs. Alstad has a Bachelor of Science degree in Marketing from Arizona State University and has specialized in procurement management for the majority of her working career. Including both retail and wholesale experience, Erin is currently the Director of Client Services for an e-procurement software company. She is in charge of an eight member team and is responsible for executing best practices to achieve the best results and exceed client expectations. Erin originally met Holly while attending Pendergast Elementary School. They were fast friends but as time went on and school districts realigned distance grew, but some friendships you never forget. In 2010 Erin attended Holly's first Fall Fling Event. As she left the event she found herself thinking about Check for a Lump and wanted to do more, give more and participate more in worthy causes. Erin is excited to be a part of the Check for a Lump team. Linda Greer, MD Member Dr. Greer is a dedicated breast imager with specialty training in diagnostic radiology. She earned her medical degree from University of Texas-Medical Branch in Galveston, TX, and completed a residency program in Diagnostic Radiology also at University of Texas-Medical Branch. Dr. Greer has extensive experience and has been a dedicated breast imager for 25 years. She has authored multiple research publications and has presented her work at many prestigious national and international conferences, including presenting a significant amount of research on Tomosynthesis (3D mammography). She is a member of several professional societies and is also board certified. Dr. Greer is committed to the advancement of early detection of disease and to providing patient-centered care to all of her patients at Arizona Diagnostic Radiology, helping them make the most informed decisions about their healthcare. Jen Umscheid Member Jen has been a part of the clinical laboratory community for 20 years and began her career in the United State Air Force as a Medical Laboratory Scientist; an opportunity that has allowed her to travel and gain a diversified experience in the world of healthcare. She currently serves as the System Quality Director for Sonora Quest Laboratories/Laboratory Sciences of Arizona based in Phoenix, Arizona and is responsible for planning, implementing, and monitoring organizational quality management strategies for 65 licensed laboratories spanning 6 states. She is passionate in her commitment to improving outcomes for the healthcare community she serves as well as supporting her amazing team! Jen also enjoys spending time with her talented husband Scott, an Executive Chef, and their rambunctious dog, Buster. Jen is grateful for the opportunity to serve and support the Check for a Lump mission because of family members personally affected. Sommer Gunia, DO Member Dr. Gunia is one of the few sub-specialty Fellowship Trained Breast Surgical Oncologists in Scottsdale and is committed to the evaluation and treatment of breast disease. She is dedicated to bringing the highest level of breast care to the valley by incorporating her expertise in the newest technologies and advancements available for both the diagnosis and treatment of breast cancer. Her training included oncoplastic surgical technique, which combines the principles of cancer removal with optimal cosmetic results. ​ After earning an undergraduate degree at Arizona State University, she obtained her medical degree from Kansas City University of Medicine and Biosciences. Dr. Gunia completed general surgery training through Ohio University and further specialized in Breast Surgical Oncology at Akron General Medical Center in Ohio. Dr. Gunia enjoys playing cards, country line dancing, relaxing by the pool with a good book, traveling, attending concerts and spending time with her husband, Steve, and their two tabby cats. Teresa Yost Member I’ve been a community volunteer for many years now. Whether it’s building homes for low-income families, gathering back to school donations, or supporting my local pet adoption center, there is always work to be done. Having years of experience in project management, I’m able to apply those skills to my volunteer work to help others whenever possible. I’ve been involved with Check for a Lump for several years now and I have met some of the most incredible individuals. The strength and perseverance these ladies exhibit is extraordinary. The stories women tell me about their journey is humbling. And the caregivers, selfless and loving. It’s important that we continue to make an impact in the fight against breast cancer and for me to be an active participant in that fight.

  • Gift a Super Survivor Kit | Check for a Lump

    Misión e Historia We offer Super Survivor Kits to women going through active breast cancer treatment. The kits include comfort items to help women during a difficult time. While we gift women going through active breast cancer in Arizona a free kit, we have decided to make our kits available to women all over the U.S. You may purchase a Super Survivor Kit to gift to your Super Survivor outside of Arizona, as well as anyone who you think could use one of our kits. Gift a Kit! About our Super Survivor Kits Each kit is made with love, community support, and generosity! The kit contains many handmade, clean, non-toxic products like: aromatherapy candles, lip gloss, hand cream, exfoliating soap, bath salts, laundry detergent, lavender infused eye masks and hot/cold pads. We even include the minimal ingredient recipes with simple, easy instructions so women can make more of the products when they run out! Also included is a reading book (either inspiring breast cancer or healing books), a product from the company Lush, coloring & game books, colored pencils, fuzzy socks, a blanket, a head cap and a hand written card. We usually include one more item, we get donations throughout the year that we are able to add into our kits, for example we are currently adding a free bra and bra fitting with a specialist, into all of our women recipient kits. Special thank you to our very own Laura Pentsa and Girl Scout Troop #7286 for making our non-toxic products and leaving a legacy of health! As we planned the Super Survivor Kits, we realized this was an amazing opportunity for our community to shine and use their talents to express their care. Our programs director, Laura began teaching her daughter’s girl scout troop the recipes. The girls loved the project and learning why non-toxic ingredients are so important. In just one night their troop made over 100 individual containers of homemade laundry detergent for the kits! The girls were overjoyed with their accomplishment and are continuing to learn more recipes to fill the kits! Our wonderful NCL volunteers have been making beautiful lavender infused eye masks. A local church, heard about the new program and within the week had bags of blankets made and ready to bring a bit of comfort. One woman heard the need and immediately left to buy fabric for the hot/cold packs. So! What has this revealed to us? People care! People want to show their support! People have many talents! And this Super Survivor Kit can be a way for all of them to collaborate with us and show each survivor that the community is there cheering them on and caring! We care about YOU!
