PROGRAMA DE PELUCAS GRATIS
Arizona pacientes con cáncer de mama sometidas a quimioterapia pueden aplicar para recibir una peluca libre. Actualmente asistimos aproximadamente a una mujer al día; ¡Más de 1,600 pelucas provistas desde el inicio!
Cómo funciona:
1. Los clientes solicitan en línea
2. Obtenga la aprobación por llamada o correo electrónico
3. Reserve una cita para la colocación de pelucas en una de nuestras tiendas de pelucas aprobadas
4. Seleccione una peluca de su elección hasta $ 250 y Check for a Lump paga la factura

Our mammogram program is open, however, due to funding restrictions we have limited availability. All screening mammograms are on a first come - first serve basis each month. The application will be open as funding allows. If you see that it is closed, please check back next month.
Anyone interested in assisting additional women by funding a free mammogram or diagnostic testing, please reach out to us! development@checkforalump.org
Screening Mammogram Eligibility and Information
Before applying to our free mammogram program, we encourage every woman to apply to the Well Woman Health Check program and/or AHCCS if you do not have insurance.
Eligibility:

Applicant is not experiencing symptoms

Applicants must live in Arizona
-
Applicants must be 35 years or older
-
Applicants must be un-insured or under-insured with financial need
-
Applicant has not had a mammogram in the past 12 months

Applicants must have a valid form of identification
-
Applicants must receive their initial screening mammogram through our program to be eligible for us to pay for additional diagnostic testing
Please bring the following information to the event:

I.D. (acceptable forms include: residency card, passport, driver license, green card)

Insurance card (if you have insurance)
-
Doctor's name and address (if you have)

Location of any prior mammogram images
Mammogram Reminder! Please do not apply deodorant or lotion to underarms or chest area the day of your mammogram.

Diagnostic mammogram information
If you are in need of a diagnostic mammogram or experiencing any symptoms, please reach out to either Valleywise Health Clinic, 1-833-855-9973, or Dignity Health Clinic, 1-855-697-0781. Both offer sliding scale fees and financial aide to those who qualify. You can also apply to the Well Woman HealthCheck program.
need follow up TESTING after a check for a lump free screening mammogram?
If you have already received a screening mammogram through the Check for a Lump program, you are also eligible for further diagnostic testing needed within one year of you screening mammogram.
