PROGRAMA DE PELUCAS GRATIS

Arizona pacientes con cáncer de mama sometidas a quimioterapia pueden aplicar para recibir una peluca libre. Actualmente asistimos aproximadamente a una mujer al día; ¡Más de 1,600 pelucas provistas desde el inicio!

Cómo funciona:

1. Los clientes solicitan en línea

2. Obtenga la aprobación por llamada o correo electrónico

3. Reserve una cita para la colocación de pelucas en una de nuestras tiendas de pelucas aprobadas

4. Seleccione una peluca de su elección hasta $ 250 y Check for a Lump paga la factura

Elegibilidad:

  • Solo atendemos a pacientes con cáncer de mama

  • Los solicitantes deben vivir en Arizona

Free Mammogram from a Local

Imaging Center:

​

1. Apply online

2. Get approval via call or email

3. Book appointment with our imaging partner

4. Receive all testing necessary for diagnosis and Check for a Lump is billed

Eligibility:

​

  • Applicants must be residents in Arizona 

  • Applicants must have a valid form of identification

  • Applicants must have a doctor's order for a mammogram

  • If you have received a mammogram elsewhere, and are in need of further testing, you will not be eligible for our program.

Free Mobile Mammogram Events

We host free mobile mammogram events throughout Maricopa County. Please view the calendar below to see all upcoming events.

Food City Free Mammogram Events:

​

On the second Thursday of every month, we host free mammogram mobile events at different Food City locations. Please view the calendar below to see when the next event in your area is! 

OPTION 2 - Mobile Event

Free Mammogram From a Mobile

On-Site Mammography:
(Must be 35+ for a Mobile Screening)

  1. Schedule an appointment for an event, please call: (480)-967-3767 or 1 (800)-285-0272. Walk-ins will fill any missed appointment slots.

  2. Go to your appoinment and receive your free mammogram.

  3. Receive mammogram results 2 - 3 weeks after screening.

  4. If you received abnormal results from your mammogram, and your doctor orders further diagnostic testing, please fill out the Mammogram Application in Option 1. 

Eligibility:​

  • Applicants must live in Arizona

  • Applicants must have a form of ID​​

  • Applicants must attend the Mobile On-site Mammography event

  • Applicants must NOT have symptoms

  • Applicants do NOT need a doctors note

Please bring the following information to the event:

  1. Insurance card (if you have insurance) and I.D.

  2. Doctor's name and address

  3. Location of any prior mammogram images

CHECK FOR A LUMP PAYS FOR 100% OF ALL MAMMOGRAMS AND ANY FURTHER NECESSARY DIAGNOSTIC TESTING.

If you have already received a mammogram through the Check for a Lump program, you are also eligible for any further diagnostic testing needed.
